for inclusion in the Directory of Clinicians
at this web site, should complete the form below and submit it.
You will be notified within 48 hours of the receipt of your application,
and within 72 hours of your acceptance or rejection for inclusion
in the Directory.
Before completing
the application form, it is necessary to read the "Terms
and Conditions." Your clicking on "I agree"
at the bottom of the Terms and Conditions
will signify that you have read, understood, and agreed to these
terms and conditions. (You will then be taken back to the application
form.)
